Care Management Services

In the course of events, your residents often need more assistance. Attending medical appointments, advocating in the emergency room, or coordinating care after a hospitalization are not services you can provide. But they are services your residents need—services that can make the difference between staying in your community or needing to move to a higher level of care.

As a care manager we can help you by:

  • Finding the right match to encourage happy placement the first time.
  • Assisting with the transition, reducing turnover for you.
  • Stabilizing chronic and serious conditions to support greater independence and well-being.
  • Prolonging the time that the resident can stay in an assisted living context.
  • Providing support when family members live far away.
  • Taking your residents to the doctor any time they need to go.
  • Being on call for emergency issues.
  • Mediating family problems.

Refer to us when you have a resident with growing needs.
Our ability to find individually-tailored solutions to even complex problems offers a real resource to the residents, staff and management at your facility. Give us a call at (804) 993-4304 to learn more.

Psychological Services

Transition trauma, behavioral challenges, and difficulties coping with functional decline and other losses are common among residents of long-term care communities and may require more emotional support and expertise than your staff are able to provide.   Successful management of these issues can sometimes mean the difference between a happy, engaged resident or an unnecessary hospitalization.

Our psychological services can help you by:

  • Reducing symptoms of depression and anxiety
  • Improving coping skills to manage stress or health conditions
  • Reducing behavioral challenges that create stress for residents, families, and staff
  • Participating with providers and care teams in developing and implementing plans of care
  • Being available to support and coach staff during a behavioral crisis
  • Supporting facility administration with compliance issues (e.g., policy development, plans of correction)
